Beneficiary Designation Forms
Simply obtain a beneficiary designation form from your IRA plan administrator, financial institution or insurer. Often these are available on the applicable company's website or through your company's human resources department.
Like a bequest, your gift remains revocable and is made only when you no longer need the assets. The following information is generally required:

If you need to provide for a surviving partner or other family member, consider naming your partner as primary beneficiary, and Animals’ Angels as secondary beneficiary.
